The digestive system is made up of elements including, among others, the mouth, esophagus and stomach. The stomach grinds the food and then pushes it into the small intestine where it is mixed with fluids secreted by the liver (bile) and the pancreas (pancreatic enzymes).  

Bariatric surgery aims to improve health and quality of life, and increase life expectancy.

3. Fax or email the form to the Outpatient Bariatric Surgery Clinic. Contact information is at the top of the form.
