The person, their family and their support network are the cornerstones for the organization of the services that contribute to the development and maintenance of their abilities, compensate for their disabilities, and enable their social participation.

Request for services

First service following a diagnosis

For patients and their families

  • Call 811 to speak to a caseworker and to open your file.

For partners and healthcare professionals

  • The referrer must fill out the Démarre le chargement du fichierreferral form (in French only) after consulting the Démarre le chargement du fichierchecklist (in French only) and attach the requested reports, or fill out Démarre le chargement du fichierAppendix 1 (in French only) for requests to the visual impairment program. For referrals related to an interim ASD diagnosis, a medical referral can also be added by completing Démarre le chargement du fichierAppendix 2 (in French only).

You do not have a diagnosis

If you are concerned about your situation or that of your child, talk to your family doctor who can refer you to the right services. You can always call 811 and speak to one of our counsellors. 

Reopening a file

Call 811 to speak to a counsellor and reopen your file. The request will be sent to the Guichet d'accès DI-TSA-DP (Intellectual disability-Autism spectrum disorder and physical disability access desk) and we will determine the orientation for the resumption of services, based on  certain criteria.

Hours : Monday to Friday from 8:30 a.m. to 12:00 noon and from 1:00 to 4:30 p.m.
