The admissions office will ask you to sign a consent to care form for treatments and possible tests. If a patient is unable to sign the form because of incapacity, a spouse, guardian or close relative must sign it.

When you come to the hospital, have your valid health insurance card and your hospital card with you. If you do not have a hospital card, the admissions staff will give you one. If you have private insurance, you must present proof of coverage for the additional hospitalization costs that will be billed to you if you choose to stay in a private or semi-private room.
