• No appointment is necessary for returning samples. 
• From 7:00 to 11:30 a.m. and from 1:00 to 2:30 p.m., Monday to Friday, except holidays. 
• Have your health insurance card and the paper requisition form with you. 
• Make sure that the requisition is fully completed: Last name and first name of the prescriber (doctor, pharmacist, nurse), their practice number and signature. 

If your requisition is missing some information, contact the prescriber to have it completed.
