Where to go

Clinique transitoire de Mont Laurier

• Located at GMF-U de Mont Laurier (757 rue de la Madone in Mont-Laurier).
• To get to the clinic, go to the GMF-U's front dest and tell them you have an appointement with the GAP clinic's nurse.

Clinique transitoire de Saint-Eustache

• Located at CLSC Jean-Olivier-Chénier (29, chemin d'Oka in Saint-Eustache).
• To get to the clinic, go reception and tell them you have an appointement with the GAP clinic's nurse.

Clinique transitoire de Sainte-Adèle

• Located at CLSC des Hauteurs (707, boulevard de Sainte-Adèle in Sainte-Adèle).
• To get to the clinic, go to the Routine health services (services de santé courants) and tell them you have an appointement with the GAP clinic's nurse.

Clinique transitoire de Saint-Jérôme

• Located at CLSC Lafontaine (1000, rue Labelle in Saint-Jérôme).
• To get to the clinic, use door 1 to get to the basement, unit 0.023. Your appointement can be with either the GAP clinic's nurse, physiotherapist or specialised nurse practitioner.
